Stratis Health’s Palliative Care in Rural Minnesota Communities
StratisHealth established this initiative to bring community-based palliative care to rural Minnesota communities. They originally supported 10 rural Minnesota communities and have sense expanded to additional communities. They say “managing a serious illness ‘take a village'”. The website has rich resources about their approach and impact.
